1. What are the income guidelines for WIC?

2. Am I eligible for WIC?

Pregnant, breastfeeding, and postpartum women, infants, and children up to age five could be eligible for WIC.

3. What do I need to bring to my appointment?

If you are being certified for WIC, you will need to bring:

• A current Medicaid card 
• Pay check stubs for everyone in the home 
• Proof of your physical address. (If your physical address is different than your mailing address, please bring proof of both.)
• Current proof of ID for you, and your child. Your child's ID can be insurance card, social security card, or for newborns, the Mother's Verification of Facts.

4. What does WIC provide?

5. I lost my eWIC card. What do I do?

Report your card as lost at 1-844-230-0813. The state will mail you a new card.

You can also stop by the WIC office to have it replaced.
